Oklahoma Permanent Makeup License Requirements
The Oklahoma State Department of Health has the following information on their website:
To apply for initial medical micropigmentation certification, you must first complete a 300 hour training program approved by the State of Oklahoma.
Then you must submit the following information:
*Complete Medical Micropigmentation Application
*Appropriate Application and Background Check Fees
*Affidavit of Lawful Presence
*Documentation of High School completion or equivalent
*Notarized copy of Birth Certificate
*copy of Driver’s License or other government-issued photo ID
*Proof of successful completion of an OSDH-approved medical micropigmentation training program (form completed by the training program/instructor)
*You may still proceed with the application process if you are not currently working and do not have a supervising physician.
